1. Recall Summary
On November 2, 2023, Lochinvar LLC, of Lebanon, Tennessee, in cooperation with the U.S. Consumer Product Safety Commission (CPSC), announced a recall of certain residential condensing boilers, combi boilers, and packaged systems. The recall includes units sold under the Lochinvar, A. O. Smith, and State Industries brand names. The recall notice was published on the CPSC website under Recall Number 24018. Approximately 17,370 units were sold in the United States, and an additional 782 units were sold in Canada. The products were sold from August 2021 through August 2023.

2. Affected Products
The recall includes residential condensing boilers, combi boilers, and packaged systems sold under the following brand names: Lochinvar, A. O. Smith, and State Industries. The brand name is printed on the front of each unit. The model number and serial number can be found on the data plate located on the unit’s rating plate, which is situated either on the inside or outside of the outer jacket of the unit. Serial numbers for recalled units fall within the range of 125325485 through 134830284.

– A. O. Smith Brand: ACB 110H-N, ACB 110H-P, ACB 110S-N, ACB 110S-P, ACB 150H-N, ACB 150H-P, ACB 150S-N, ACB 150S-P, ACB 199H-N, ACB 199H-P, ACB 199S-N, ACB 199S-P.
– State Industries Brand: SCB 110H-N, SCB 110H-P, SCB 110S-N, SCB 110S-P, SCB 150H-N, SCB 150H-P, SCB 150S-N, SCB 150S-P, SCB 199H-N, SCB 199H-P, SCB 199S-N, SCB 199S-P.

The products were sold by independent contractors, plumbers, and plumbing supply vendors and distributors from August 2021 through September 2023 for between $4,000 and $10,000, including installation. A limited number were sold to original equipment manufacturers who modify and use the boilers in integrated in-floor heating systems. The units were manufactured in the United States.

3. Hazard Information
The recall notice states that the boiler’s flue collector can crack if exposed to a high impact event in the burner chamber. This condition can allow the boiler to emit carbon monoxide, posing a risk of carbon monoxide poisoning. According to the recall notice, Lochinvar has received nine reports of broken flue collectors. Lochinvar has received no reports of injury.

4. Consumer Action Required
Consumers with the recalled boilers should immediately contact the installer or a qualified technician to schedule a free repair. Consumers who continue use of the boilers while awaiting repair should have working carbon monoxide alarms outside of sleeping areas and on each floor of the home.

5. Official Remedy
The official remedy is a free repair. The repair will consist of installing a steel plate, which will be affixed to the base of the flue collector to act as a reinforcement.
