1. Recall Summary
Fisher-Price Inc., in cooperation with the U.S. Consumer Product Safety Commission (CPSC) and Health Canada, is recalling the tissue box toy sold with the Fisher-Price 3-in-1 SnugaPuppy Activity Centers. The recall was initiated due to a choking hazard. The recall notice was published on April 10, 2025.
2. Affected Products
The recall involves the tissue box toy that is sold as a component of the Fisher-Price 3-in-1 SnugaPuppy Activity Center. The activity center’s model number is HLV78, which is located on the underside of the table, near the Fisher-Price logo. The recalled tissue box toy is white with red decorations. It contains “tissues” made of a sensory cloth with a black and white wiggly striped pattern on one side, and a yellow/green pattern with raised spots on the other side.
The products were sold at Walmart stores nationwide, and online at Amazon.com, Walmart.com, and other websites from November 2022 through February 2025. The activity center was sold for approximately $130. The number of units affected is about 15,300 in the United States. In addition, about 2,000 units were sold in Canada. The product was manufactured in China.
3. Hazard Information
The hazard associated with this product is that the detachable tissue box toy can come apart, exposing small support brackets. This poses a choking hazard to young children. According to the recall notice, Fisher-Price has received one report of the tissue box toy coming apart and an infant placing the small support bracket in their mouth. No injuries have been reported.
4. Consumer Action Required
Consumers are instructed to immediately stop using the recalled tissue box toy. The toy should be removed from the activity center and kept away from children. Consumers must then contact Fisher-Price to receive a free replacement toy for the activity center. As part of the remedy process, consumers will be asked to mark the tissue box toy permanently with the word “Recall” and a unique identifier. Consumers must also mark an “X” on the sensory cloth tissues. After marking the product, consumers must upload a photo of the marked toy to the designated recall website. Upon receipt of a replacement confirmation email from Fisher-Price, consumers should dispose of the recalled product in the trash. Fisher-Price is contacting all known purchasers directly.

Consumers are not receiving a refund or a replacement for the entire activity center; the remedy is specifically for the tissue box toy component. The replacement is provided after the consumer completes the marking and photo upload process as described in the consumer action instructions.
